Consultation requests
Submitting a consultation request does not create a patient-provider relationship or guarantee an appointment. Please provide accurate contact information so our team can respond. Do not include detailed medical history or other sensitive health information in the website form.
SMS terms
If you enter your mobile number and check the optional SMS consent box, Tyler Prosthetics may send transactional text messages confirming your consultation request and following up about that request.
Message frequency varies. Message and data rates may apply. Reply STOP to opt out at any time. Reply HELP for help.
